POST:                                    Support Worker

RESPONSIBLE TO:           Project Manager & Project Support Officer

PURPOSE:

To provide housing related support to enable residents to maintain their tenancies and to achieve their aspirations for independent living.

AREAS OF RESPONSIBILITY:

• To identify residents individual support needs and through consultation and agreement with residents, devise and implement suitable support plans to ensure that identified support needs are met
• To carry out risk assessments as and when necessary due to changing needs and circumstances of the residents
• To participate in the assessment and selection of tenants ensuring that good practice and equality of opportunity is followed
• Advocate for residents where appropriate
• Organise and provide appropriate support for residents preparing to move into greater independence, including support through the move and resettlement into their new home
• To promote and comply with Supporting People standards and Magna's Equality and Diversity, Data Protection and Health & Safety policies and procedures.
• Contribute to the protection of service users from abuse and support residents when in distress
• Liaise with other housing, health and social care professionals, benefits, welfare and advocacy agencies as appropriate
• Work within a team, supporting colleague and attending team meetings, recording and sharing information appropriately and confidentiality.
• To promote and develop resident involvement and responsive customer services
• To undertake all core training required for this post and other training and professional development activities as requested

The Support Worker may be asked to carry out other duties which are broadly consistent with those detailed in this Job Description.
